I suspect Firefox uses a hash map to handle the cached images. This method is quick, but on occasion there will be 'collisions,' where two keys are mapped to the same location in the hash table. This is probably what occurred with these two images. One overwrote the other, and when Firefox tried to retrieve the first image from the cache, it got the second instead, not realizing they were different.

That's just a guess, though.
